Start & Maintain Patients on CAMZYOS® With 3 Steps
Enroll
Only a REMS-certified provider can prescribe CAMZYOS.
- Enroll yourself in the CAMZYOS REMS program (one time only)
- Enroll a Designee (optional)
- Complete and submit the REMS Patient Enrollment Form
- Patient and HCP signature required
- Enroll your patient in the MyCAMZYOS Patient Support program (optional and only after the treatment decision has been made)
REMS CERTIFICATION: VISIT CAMZYOSREMS.com
Certification can be completed online at CAMZYOSREMS.com or by printing and faxing the HCP Enrollment Form to 833-299-9539. HCPs will be notified if they have been certified to prescribe CAMZYOS within 1 business day.
PATIENT EDUCATION & ELIGIBILITY
Review and provide the following materials to educate your patients on CAMZYOS:
- REMS Patient Brochure (required)
- CAMZYOS Patient Brochure
Prior to initiating treatment, confirm your patient’s eligibility and assess baseline clinical status:
- Absence of pregnancy and use of effective contraception
- LVEF is ≥55% via echo
- Review patient’s medicines, including OTC medications and supplements, to assess for DDIs or contraindications
Prescribe
CAMZYOS is only available from REMS-certified specialty pharmacies.
If your patient is enrolled in MyCAMZYOS:

If your patient is not enrolled in MyCAMZYOS:
- Complete and submit prescription to your patient’s insurance-mandated specialty pharmacy
- Submit prior authorization form if required
STAY CONNECTED
Remind your patient to answer correspondence or communications from their specialty pharmacy and MyCAMZYOS (if enrolled).
Due to their own REMS requirements, specialty pharmacies will also reach out to the HCP for clarifying questions.
Maintain & Monitor
Timely echo scheduling is essential to support uninterrupted treatment.
After the initial prescription:
- Confirm and record your patient’s start date in the CAMZYOS REMS portal
- Submit prescriptions to your patient’s specialty pharmacy
For subsequent prescriptions:
- Schedule you patient's initiation phase echos and clinic visits at Week 4, 8, and 12, and their maintenance phase echos and clinic visits every 3 or 6 months, based on individual echocardiographic parameters
- Conduct a clinical assessment, including a review of medications, after each echo or as needed
- Echo findings will inform any changes or treatment interruptions, which may require additional echos

- Submit Patient Status Form in the CAMZYOS REMS portal within 3 days after each echo is due to support timely dispense of medication
